Agreement to Terms

By scheduling an appointment, receiving treatment, or using our website, you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our services.

Dental Services

Lina E Vega General Dentist PLLC provides general and cosmetic dental services. All treatments are provided by licensed dental professionals. Treatment plans are developed based on individual patient needs and may vary based on clinical findings.

Appointments

  • Scheduling: Appointments can be scheduled online, by phone, or by email. All appointments are subject to availability.
  • Confirmation: We will confirm your appointment within 24 hours of your request.
  • Cancellations: Please provide at least 24 hours notice for cancellations or rescheduling.
  • No-Shows: Repeated no-shows may result in a scheduling fee or dismissal from the practice.
  • Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule.

Patient Responsibilities

As a patient, you agree to:

  • Provide accurate and complete medical and dental history
  • Inform us of any changes to your health status
  • Follow treatment recommendations and post-care instructions
  • Arrive on time for scheduled appointments
  • Maintain good oral hygiene between visits
  • Notify us of any concerns or complications promptly

Financial Terms

  • Payment: Payment is due at the time of service unless other arrangements have been made.
  • Insurance: We accept most major dental insurance plans. Patients are responsible for understanding their coverage and any amounts not covered by insurance.
  • Estimates: Treatment cost estimates are provided in good faith but may change based on actual clinical findings.
  • Collections: Outstanding balances may be sent to collections after 90 days.

Informed Consent

Before any treatment, we will explain the procedure, risks, benefits, and alternatives. You will be asked to provide informed consent before treatment begins. You have the right to ask questions and decline treatment.

Emergency Care

We reserve time for dental emergencies and make every effort to see emergency patients the same day. If you experience a dental emergency outside of office hours, please go to the nearest emergency room or call our emergency line.

Records and X-Rays

Your dental records, including X-rays, are maintained in accordance with state and federal regulations. You may request copies of your records at any time. A reasonable fee may apply for record copying.

Limitation of Liability

While we strive to provide the highest quality care, dental treatments carry inherent risks. We are not liable for complications that may arise despite proper treatment and care. Our liability is limited to the extent permitted by applicable law.

Governing Law

These Terms and Conditions are governed by the laws of the State of New York. Any disputes shall be resolved in the courts of Monroe County, New York.

Changes to Terms

We reserve the right to modify these Terms and Conditions at any time. Changes will be posted on our website and will be effective immediately upon posting.
